Getting patients started on one of the FreeStyle Libre CGM systems is easy*

Prescribing instructions

For commercially insured and cash-pay patients

FreeStyle Libre 3 system Rx includes:

  • FreeStyle Libre 3 app

  • FreeStyle Libre 3 sensor

    • Quantity: 2 sensors/month NDC# 57599-0818-00
    • Sensor refills: PRN or 12 refills annually

FreeStyle Libre 2 system Rx includes:

  • FreeStyle Libre 2 app§

  • FreeStyle Libre 2 sensor

    • Quantity: 2 sensors/month NDC# 57599-0800-00
    • Sensor refills: PRN or 12 refills annually

Patients can fill their Rx at a participating pharmacyǁ with their choice of either FreeStyle Libre 2 or FreeStyle Libre 3 system.

If your commercially insured patients are asked to pay >$75 for two sensors or have questions about coverage and costs, please have patients call 844-330-5535.

For Medicare patients

  1. Determine if patient’s coverage is Medicare Fee-For-Service or Medicare Advantage

  • Medicare Fee-for-Service: Refer to CMS Continuous Glucose Monitors coverage criteria and documentation requirements to confirm coverage

  • Medicare Advantage: Contact the patient’s insurance for eligibility criteria

  1. FreeStyle Libre 2 DME Supplier Grid provides options to select a DME#

  2. Complete Medicare Detailed Written Order form with the patient’s information

  3. Explain to the patient they should expect a call from the DME

The prescription must include both the reader and sensors to be eligible for Medicare coverage. At this time, the FreeStyle Libre 3 system is not eligible for Medicare reimbursement**.

The FreeStyle Libre apps, sensors, and readers cannot be used interchangeably across the family of products.

Get your patients started with an app‡§

Have your patient download the corresponding FreeStyle Libre 3 app or
FreeStyle Libre 2 app§ and check to see if their smartphone is compatible.

For patients without a compatible smartphone‡§

  • 1 FreeStyle Libre 2 reader

  • 1 USB cable

  • 1 power adapter

  • Treatment Decisions Guide (a guide to help patients use reader information to make informed treatment decisions)

  • Interactive Tutorial USB Card (a comprehensive electronic resource on how to operate the FreeStyle Libre 2 system, including instructional videos)

The FreeStyle Libre 2 reader is only compatible with FreeStyle Libre 2 sensors. No standalone reader is available for the FreeStyle Libre 3 system.

Medicare coverage is available for the FreeStyle Libre 2 system if the FreeStyle Libre 2 reader is used to review glucose data on some days every month. Medicare and other third party payor criteria apply.

At this time, FreeStyle Libre 3 is not currently eligible for Medicare reimbursement, and Medicaid eligibility may vary by state.

† Based on a comparison of list prices of the FreeStyle Libre personal CGM systems versus competitors’ CGM systems, assuming annual use of one receiver (or equivalent hardware) and quantity of transmitters and/or sensors according to use life. The actual cost to patients may or may not be lower than other CGM systems, depending on the amount covered by insurance, if any.

†† The FreeStyle Libre 2 app and the FreeStyle Libre 2 reader have similar but not identical features. Fingersticks are required for treatment decisions when you see the Check Blood Glucose symbol and when your glucose alarms and readings from the system do not match symptoms or expectations.

FreeStyle Libre Pro: The FreeStyle Libre Pro Flash Glucose Monitoring System is a professional continuous glucose monitoring (CGM) device indicated for detecting trends and tracking patterns and glucose level excursions above or below the desired range, facilitating therapy adjustments in persons (age 18 and older) with diabetes. The system is intended for use by health care professionals and requires a prescription.

IMPORTANT: The device may inaccurately indicate hypoglycemia. The results of the clinical study conducted for this device showed that 40% of the time when the device indicated that user sensor glucose values were at or below 60 mg/dL, user glucose values were actually in the range of 81-160 mg/dL. Therefore, interpretation of the FreeStyle Libre Pro Flash Glucose Monitoring System readings should only be based on the trends and patterns analyzed through time using the reports available per the intended use.

CONTRAINDICATIONS: Remove the Sensor before MRI, CT scan, X-ray, or diathermy treatment.

WARNINGS/LIMITATIONS: The FreeStyle Libre Pro System does not provide real-time results and patients should adhere to their blood glucose monitoring routine while using the system. If a sensor breaks, contact physician and call Customer Service. Patients with high levels of ascorbic acid (Vitamin C) or salicylic acid (used in Aspirin) or severe dehydration or excessive water loss may experience inaccurate results with this system. The FreeStyle Libre Pro System is not approved for pregnant women, persons on dialysis, or recommended for critically-ill population. Sensor placement is not approved for sites other than the back of the arm and standard precautions for transmission of blood borne pathogens should be taken.
