Prescribe the FreeStyle Libre systems—the continuous glucose monitoring (CGM) system that’s more affordable for your patients*1.

Prescribe two FreeStyle Libre 3 Plus or 2 Plus sensors [30-day supply, filled monthly; sensor refills: PRN or 12 additional refills (1-year supply)].

Have your patient download the corresponding FreeStyle Libre systems apps†‡.

Patients can fill their prescription for:

  • FreeStyle Libre 3 Plus sensor: At participating pharmacies§
  • FreeStyle Libre 2 Plus sensor: At participating durable medical equipment (DME) suppliersII

If your commercially insured patients are asked to pay >$75 for two sensors,
or if they have questions, please have patients call 844-330-5535.​

Most commercially insured patients save over 50% every month for their FreeStyle Libre 3 or 2 sensors with an approved prior authorization (PA)¶1.

The FreeStyle Libre systems apps†‡, sensors, and readers cannot be used interchangeably across the family of products.

See if your patient is covered under Medicare Fee-for-Service or Medicare Advantage using the CMS Continuous Glucose Monitors coverage criteria#2.

For Medicare patients, complete a Standard Written Order Form.
 

Let patients know they can expect a call from the DME if any additional information is needed. Once approved, the product will be shipped to their home.

For questions regarding Medicare Fee-for-Service coverage, please visit www.medicare.gov.

To be eligible for Medicare coverage, the prescription must include both the reader and sensors, and the reader must be used on some days every month.
