How to identify patients

Here’s what to consider when selecting patients who can benefit from CGM with FreeStyle Libre systems.

 
 

1. Make a diagnosis6:

  • Patient is diagnosed with Type 1 or Type 2 diabetes

2. Note any clinical considerations6 that apply to your patient:

  • Not achieving glucose targets
  • High degree of glycemic variability
  • Frequent hypoglycemia
  • Hypoglycemia unawareness
  • Taking multiple daily injections of insulin
  • Using an insulin pump

3. Consider cost and coverage options for your patient, including:

  • Commercial insurance
  • Medicare coverage
  • Medicaid coverage
  • Veterans Affairs or TRICARE coverage
