Practitioner acknowledges that, at their request, Abbott Diabetes Care (ADC), located at 1360 South Loop Road, Alameda, CA 94502, is providing FreeStyle Libre product(s) at no charge to Practitioner solely for distribution to patients in accordance with the indications for use.

Practitioner represents and warrants that they are licensed by applicable state law(s) to use or order the use of prescription medical devices in the course of their professional practice. To the extent that the Practitioner’s prescribing authority is predicated on meeting additional state requirements, including, but not limited to, certification from the relevant professional board authorizing prescriptive authority or a collaboration/supervision agreement with a physician that addresses prescriptive authority, they represent and warrant that they comply with any additional requirements.

Practitioner certifies that they will not resell or trade the no-charge products(s) and will not seek reimbursement from any patient or third-party payor, including federal health care programs. If the services provided by the Practitioner are covered under global payment systems (such as DRGs), then the no-charge product(s) must be treated appropriately under the billing procedures applicable to the payment system.

Practitioner acknowledges that the FreeStyle Libre systems no charge product(s) are for trial use only. Practitioner certifies that they will require each patient to discard/dispose or return any FreeStyle Libre systems no charge product(s) at the conclusion of the patient’s wear experience of FreeStyle Libre Systems no charge product.

Practitioner certifies that they are aware that the FreeStyle Libre no-charge product(s) are not approved for inpatient use or for patients in acute care who are critically ill and is provided by ADC for intended use in accordance with product labeling.

What is web sampling§?

This is an invitation-only program through which Abbott's Diabetes Care team is providing healthcare providers samples of FreeStyle Libre systems sensors, to enable more patients with diabetes to experience the benefits of the FreeStyle Libre systems.

Who is eligible for web sampling§?

Healthcare providers who participate in Abbott’s email education program may be eligible to request samples of FreeStyle Libre systems sensors.

Government employees are not eligible to receive free samples from Abbott.

The sensor samples are intended for patients with diabetes who can benefit from a continuous glucose monitor.

* Study was performed with the outside US version of the FreeStyle Libre 14 day system. Data is applicable to FreeStyle Libre 2 and 3 systems, as feature sets are similar as FreeStyle Libre 14 day system, excluding alarms.

† Based on retail and durable medical equipment (DME) sales data for patient’s last-filled prescription, by manufacturer.

‡ Based on prescription claims for the aggregate of patients covered by Commercial insurance, Managed Medicare, Managed Medicaid using the FreeStyle Libre personal CGM systems versus competitors’ CGM systems.
